Patient Details
This is a 20 year old male, 6’2″ and 165 pounds who presented with gynecomastia or puffy nipples. On examination he had a fair amount of glandular tissue present on both sides. The cause of his gynecomastia is idiopathic or “unknown”. It has been present since puberty. It affects him tremendously and he has paid a high price for having this condition. He is seen here after a bilateral gynecomastia surgery removal of mostly glandular tissue with some liposuction “feathering” of the periphery as well as the use of my internal flaps. He is seen here one year out from his treatment.
He is as happy as ever. He tells me that he doesn’t even think about it anymore and feel amazing relief. It has changed his life for the better.
Lessons to learn:
- Check out the amount of tissue that was removed from his chest area. It’s hard to believe that all this tissue was in his chest!
- Puffy nipples are caused by tissue beneath the areola’s. Since it has been removed, the areola’s are now flat. I performed my “turnover” flaps as I felt he needed additional support beneath the nipple-areola complexes or it may have looked somewhat “caved in”. This flap comes in handy as it allows me to move all or almost all of the glandular tissue while replacing support under the areola with normal tissue. I started using this flap in 1997.
