Patient Details
This is a 52 year old mother of three who is 5’7″ and 140 pounds who had multiple previous breast procedures done elsewhere. She told me she is “tired of these things”. It appears that her complaints are all about the general size of her breasts—they are too large, too heavy and she gets back and neck pain and they feel “uncomfortable”.
We discussed options, and what she wants is higher, smaller and tighter breasts. She wants a size that fits her figure and is more comfortable.
She is seen here about seven months after a breast revision. I removed 475cc saline implants in the subglandular position and replaced 350cc high profile silicone implants in the same pocket. I did a small breast reduction along the lower part of her breasts and lifted her breasts through a vertical approach with the final scar being shaped in a lollipop fashion.
She is very happy with her result. Her breasts are not smaller and perkier for sure, and she is very happy with the new proportion that exists.
Lessons to learn:
- I would have liked to put her implants beneath the muscle where she thought they were but the muscle was too retracted from her previous surgeries. More coverage of the implant by tissue means a better, more natural result. I can actually feel her implant along the midline because there is not a generous layer of tissue covering the implant. There is nothing that needs to be done per se.
- It’s not uncommon for women to request smaller “simpler” and more manageable breasts when they get older. It may have been more fun to have larger implants when young, but as she aged the weight of the breasts becomes a drag.
- I know that she had given up on her breasts before the surgery and I know now that she really loves her new (revised) assets. This is a good thing. We are really happy for her.
