Patient Details
This is a 23 year old young woman who is 5’3″ and 125 pounds who was interested in a breast augmentation. She knew what she wanted-425cc implants. She spent a lot of time looking at before and after pictures and knew what she wanted.
On examination, she was fit and trim with nice, albeit modest sized breasts. There was a significant gap between her breasts. The shape was excellent and her tissues were of high quality. She is seen here after a bilateral augmentation with 425cc high profile silicone implants placed beneath the muscle through an incision at the bottom of the breasts. She is happy with her results. She wears an OK bra at best. There is some lateral displacement of her breasts when she lies on her back. She never really expanded her breast pockets medially like I told her because her husband really didn’t help work them much after her procedure. I told her to learn how to wear a supportive bra as she clearly told me that she doesn’t like how constricting supportive bras feels and she likes sexy bras.
Learning points:
- Don’t get me wrong; her breasts are beautiful but I would have liked to see how much we could have moved them medially for more cleavage and closer breasts.
- If someone doesn’t work her breasts towards the midline to expand the pocket it won’t go there. Failure to move them most commonly occurs when it’s simply not happening because the patient can’t do it for themselves and the “helper” simply isn’t doing his/her job. It’s easy for me to tell when the work isn’t getting done as there is no progress over several weeks. If you mold the breasts on a daily basis, several times a day for up to an hour at a session, I guarantee that they will move to the right area. Cleavage happens for some because of the shape of their breasts allow for it easily. For others with separated breasts, YOU NEED TO CREATE IT WITH BREAST MOLDING.
- Wearing a supportive bra does not mean torture or bondage. It means finding the right bra that does the work. It doesn’t need to be an underwire bra. It simply needs to be a bra that does the work. You may have to get professionally fit or you simply need to know what you are looking for and try on a bunch until you find one that works. A sexy bra can also be a “working bra”.
