This patient is 5 feet tall and weighs 160 pounds. She has two kids and has struggled with her weight. She said she simply didn’t have the body that she used to. We had very personal and intense discussions about her weight and the benefits and risks of surgery. I felt she was a fair candidate for a first stage liposuction procedure followed thereafter possibly by an abdominoplasty to remove excess skin and tighten the fascia. On examination I found her skin to be only of fair quality and that she had intra-abdominal fat that made her abdomen protrude more than usual. This fat could only be reduced with weight loss–liposuction and a tummy tuck will not completely flatten her abdomen because part of the problem is the fat on the inside of her abdomen (visceral fat).
She is seen here five months after a first stage liposuction where I removed 8500cc of fat (a large volume liposuction). She is now 148 pounds and is remarkably smaller and more shapely. Her abdomen is flatter not only because of her liposuction but also as a result of fat loss from inside her abdomen because of the additional weight loss. Her skin has some irregularities as you would commonly expect when you have 50 year old skin. She notes that it’s more difficult to be disciplined about weight loss the further out from surgery she is but clearly understands the need for continued weight loss. The struggle with weight doesn’t change after plastic surgery; it just seems easier when you enjoy the benefits of your investment. It’s easier to be motivated when you look and feel good.
