Patient Details

This is a 21 year old mother of one, who is 5’6″ and 136 pounds interested in a breast enhancement. On examination, she has asymmetrical breasts and asymmetrical ribs. She has a “funnel chest” shape to her chest as well (the central or chest bone area is depressed). She has a “go big or go home” attitude regarding her breast size.

She is seen here after a bilateral below the muscle breast augmentation with a 400cc high profile silicone implant on the left and a 450cc implant on the right side. She is extremely pleased with her result and is not interested in any lifting procedures at this time.

Learning points:

1. This is not a straight forward case. Do we lift or just enhance alone without a lift? How do we compensate for her chest wall and breast shape asymmetries? In her case she wanted larger implants. This is good because it will fill her chest out better and provide for some enhanced “lift”. We chose different sized implants to compensate for the breast/chest differences.
2. Her result is not perfect, but it is wonderful. There is much improved symmetry and the larger implant has done a wonderful job of filling in her chest depression on the right side.
3. Now that her breast “foundation” has been set with her breast implants, a “lift”, if needed, is straight forward now.
4. Many years’ experience with the breast enhancement process leads me to conclude that patients are happy/unhappy with certain features of their results. It’s not always obvious what this is, but it is obvious when I observe my patients body language afterwards. I think in her case it’s the size, increased symmetry and general overall improvement in her “condition”. I don’t think she cares that much that her breasts are not perfectly symmetrical or the differences in the shape/location of her nipple-areola complexes. I don’t have a “cookie cutter” approach to plastic surgery. Each individual is unique. My job is to find out what they prefer and try to take them to that outcome.
