This is a wonderful story of a relatively young woman who lost a significant amount of weight, resulting in a dramatic change in her breasts. With massive weight loss, breast tissue often loses volume and collapses, leaving the breasts deflated, sagging, and poorly supported. Unfortunately, this is not uncommon—and it represents one of the most challenging problems in plastic surgery.

In these cases, the breast tissue hangs low on the chest, the nipple–areola complexes point downward, and the skin quality is severely compromised. Attempting to “lift” this tissue alone is unrealistic. It’s like trying to build a two-story house out of straw—it simply doesn’t have the structural integrity to hold its shape long-term.

This is precisely why my approach—breast augmentation combined with breast reduction—is such a powerful and effective solution. The breast implant is used to create the new breast form and place it higher on the chest where it belongs. The breast reduction component removes the damaged, stretched tissue that cannot be reliably lifted or reshaped. You cannot take poor-quality, hanging tissue and suspend it into a perky position and expect it to last—no matter what technique is used.

This approach is often referred to as the plus–minus procedure:

• The plus is the implant, which establishes the breast’s shape, projection, and position
• The minus is the removal of damaged, sagging tissue

The result is a breast that is firmer, perky, and optimally positioned on the chest wall—something that lifting alone cannot achieve in these complex cases.

This is not a procedure commonly performed by most surgeons. Many have never been exposed to the technique, while others were taught that it is too difficult or too risky.
