This is an interesting case. This patient has tubular shaped breasts and presented for what she thought was a routine breast augmentation. However, this is far from the reality of the situation. She had widely spaced breasts with a tubular breast shape and a narrow/small breast footprint. She wanted the opposite appearance—larger, fuller breasts with cleavage and a nice shape.
After lots of education, we decided to use 345cc smooth walled silicone implants in the subglandular position with liposuction contouring of her lateral breast/chest areas and splaying of her breast tissue all done through a small incision at the bottom of her breasts. She had to work her result by molding her breasts around her new implants and she did a very impressive job. This result says it all—beautiful breasts that fit her body with an impressive non tubular shape. The liposuction on the lateral breast areas was critical in the more youthful appearance of her chest/breasts. She is very pleased with her investment. There were all sorts of issues that could have appeared based on her anatomy but we were able to achieve our objectives because of my experience with challenging breasts such as hers. Now it’s time to enjoy her investment and preserve her breast shape with an excellent bra more often than not.
