This patient had undergone three previous breast operations, the most recent being placement of 425 cc breast implants. Like many women I see, her concerns were not related to a surgical complication but rather to a change in her priorities over time. As she told me, "When I was twenty, this wasn't a problem, but now I'm older."
Her implants had become heavy, made her figure appear more matronly, and no longer reflected how she wanted to look or feel. She had simply grown tired of having breast implants.
Equally important was the anxiety she developed after spending countless hours reading information online. Like many women seeking consultation today, she was understandably confused by the enormous amount of conflicting information regarding breast implant removal, implant capsules, Breast Implant Illness (BII), and the very rare lymphoma associated with certain textured breast implants.
After discussing the available scientific evidence, I recommended a straightforward implant removal while leaving her implant capsules in place. This is the most common approach in my practice because, in the absence of a specific medical indication such as significant capsular contracture, implant rupture requiring capsulectomy, or suspected malignancy, removal of the capsule provides no proven benefit and only increases the extent and risk of surgery. My philosophy is simple: perform the least invasive operation necessary to safely achieve the patient's goals.
Following implant removal, she reported feeling better overall. Interestingly, she described the improvement as modest rather than dramatic. This is consistent with what I often observe. For many women, there is tremendous peace of mind in no longer carrying implants that had become a source of worry. Whether that improvement results from physical changes, psychological relief, or a combination of both is difficult to determine, but what matters most is that she feels more comfortable in her body and confident in her decision.
The goal of breast implant removal should not be driven by fear, but by thoughtful evaluation, accurate information, and an operation tailored to the individual patient rather than internet misinformation.
