Pectus excavatum presents one of the more challenging situations in aesthetic breast surgery. In this condition, the center of the chest wall is caved inward and the rib cage is often significantly asymmetrical from side to side. These underlying structural differences can make achieving breast symmetry considerably more difficult, even before surgery begins.
This patient desired fuller, more youthful breasts but also required reshaping to address breast laxity and asymmetry. Careful preoperative planning was critical because her chest wall differences strongly influence implant positioning, projection, and overall balance.
To improve her shape, I placed 400 cc smooth silicone implants beneath the muscle and then performed a bilateral vertical mastopexy (breast lift) to reshape and tighten the breasts around the implants. The goal was not simply to enlarge the breasts, but to create a result that looked natural, proportionate, and balanced with her figure while disguising the underlying thoracic asymmetries as much as possible.
At approximately 3 months after surgery, her outcome is already quite impressive. The implant size works beautifully with her frame, her breasts are significantly more symmetrical, and the implants help camouflage the irregularities of the chest wall exceptionally well. Her overall breast shape is youthful, balanced, and feminine.
Importantly, perfection is never the expectation in patients with significant chest wall deformities. Rather, success comes from understanding the anatomy, anticipating the limitations, and creating the best possible aesthetic balance. In her case, the improvement in symmetry and contour exceeds what most reasonable patients would expect given the complexity of her anatomy.
