Patient Details

The diagnosis was straightforward: the patient had cephalically oriented lower lateral cartilages, meaning the lateral components of her tip cartilages were rotated upward toward the head rather than positioned laterally along the nostrils. In her case, these paired cartilages were even visible through the skin.

This anatomic issue created a nose that appeared elongated with a bulbous, poorly supported tip. The nostrils lacked proper support because the cartilage framework was rotated upward instead of providing lateral structure.

During surgery, I reoriented the malpositioned tip cartilages to restore proper support and shape. I also made refinements to the dorsum and nasal bones to create a smoother, more natural overall appearance.

Although these photos are only 7 weeks post-operative, the improvement is already evident—even with expected residual swelling in the tip. As the swelling continues to resolve, particularly over the next 6–12 months, the result will become more refined and balanced.
