More than 10 years after her BRA procedure—breast reduction combined with augmentation (450cc silicone implants)—this patient remains delighted with her breasts. She especially appreciates how they continue to sit high on her chest with lasting upper breast fullness.
Her daughter’s experience has been different. After undergoing a breast lift with implants elsewhere, she came to me disappointed with significant recurrent sagging.
These experiences illustrate an important consideration: lifting a breast does not necessarily address the heavy, poorly supported tissue contributing to its sagging. In patients with substantial sagging breast tissue, I believe reducing that tissue is essential to creating a more durable shape.
My BRA procedure removes excess lower breast tissue and uses an implant to provide upper breast fullness. The goal is to shift the breast’s footprint upward for a more youthful, flattering shape. Breast reduction does not necessarily mean a smaller final breast. Implant selection allows us to tailor the size to the patient’s goals—smaller, similar in size, or larger.
As I often explain, tissue that has been removed cannot sag again. The remaining breast tissue and skin will continue to age, however, and no procedure guarantees permanent freedom from sagging. This patient’s result demonstrates what is possible more than a decade later.
Simultaneous breast reduction and augmentation is a complex operation that is less commonly performed than a breast lift with implants. Experience with this specific combination matters. Before choosing surgery, consult several reputable, accomplished, board-certified plastic surgeons. Ask how they would address your sagging tissue and request long-term results in patients with anatomy similar to yours.
